Signing Forms

This video outlines the steps for Student-Athletes to complete requested forms.

1. Login to the Student-Athlete portal
2. Once you have logged in to the Student-Athlete portal, you will be asked to Confirm or Update your contact information
3. Click on the form name under My Inbox to open the form
4. Fill out and complete the form in its entirety
   • Note: Required fields are highlighted yellow on the form
5. Enter your Institutional ID number (UIN) and click Sign Form

 Note: If you are under the age of 18 then you will be required to upload a parent or legal guardians signature for several NCAA forms (eg. NCAA Drug-Testing Consent). Follow the steps below to upload a parent or legal guardians signature to the forms if you are under the age of 18:

1. Complete the form in its entirety
2. Print the completed form using the Print icon in the upper right corner of the screen
3. Have your parent or legal guardian sign the completed form 
4. Attach the signed copy using the Upload File button (Tip: To obtain a digital version of the signed form you should 1) Scan the signed copy of the form and attach OR 2) Take a picture of the signed form and attach)
5. Click the Sign Form button to complete the form 

 Note: If you are under the age of 18 you will NOT be able to complete the NCAA forms without uploading an attached parent/guardian signed form
