Adding Questionnaire Header

Adding a banner to your online questionnaire is a great way to personalize the questionnaire for your sport program. We recommend that the file be no larger than 850 pixels wide by 350 pixels tall and be in a .jpg or .gif format. Once you upload the banner, it will be located along the top of the online questionnaire.

1. Click Top Right Menu
2. Click on Questionnaire
3. Click on Questionnaire Settings
4. Scroll to the bottom until you see Banner Image
5. Click on Choose File to upload the new picture file
6. Select the picture (.jpg or .gif) file 
7. Click Open
8. Click Save & Return
